– Home Services Infusion Nursing that will work in collaboration with the Infusion Registered Nurse (RN) in planning and delivery of patient care and to provide and/or assist with provision of individualized and specialized care of patients within the ambulatory infusion suite and/or within the community setting.

The Infusion RN facilitates the creation of an individualized plan of treatment for each patient that is instituted in accordance with physician orders and therapy goals.

The Infusion LPN provides ongoing infusion related care and follow-up in accordance with the plan of treatment, physician orders/prescriptions and outcome goals while also educating patients in promotion of self-care.

Successful outcomes include superior quality and safety metrics, exemplary customer service, outstanding patient satisfaction and documented success in achieving patient outcome goals.

The Ambulatory Infusion LPN acts as a communication link between Providence Infusion Pharmacy Services, clinicians, patients, families, and physicians by creating a user friendly point of entry to discuss questions, problems or concerns throughout the continuum of care.In this position you will have the following responsibilities:Complete assessment of physical, psychological and social dimensions of patients, following verification of assessment skillsParticipate in formally greeting and welcoming patientsInterview patients, records vital signs and weights, provides clinical monitoring, schedules/outlines follow-up visits, ensures completion of visit-related paperwork including signatures from patients/representatives, utilizes, reviews and retrieves as appropriate clinical information from the medical record(s) promptly and accurately documents findings in the medical recordDirectly participates in implementation of outlined elements within the plan of treatmentFunction within the limits of the practical nurse law, in home health regulations as applicable, ministry/HCC policies and procedures, local/regional and national regulations, Joint Commission and individual skills and competencyAssist in evaluation of nursing care; questions and clarifies procedures, treatments, and medications that may not be appropriate for the patients physical/mental status at that timeThe Licensed Practical Nurse may administer medications via all routes with the following requirements related to intravenous/injectable medication administration.

Intravenous/Injectable medication may be administered by an LPN if they are competent in that skill.

Administration may occur on a case by case basis for non-documented competencies under the direct supervision of an RN.

Defer/report nursing issues outside their scope to the RN Supervisor/ManagerAssist by providing input and information to the RN in planning a formal written plan of treatment for patientsPerform treatments, or assists in treatments and procedures following verification of skills via the skills/competencies checklistPerform specialized procedures under the direct/indirect supervision of the RN.

Examples of nursing treatments that require specialized nursing skills include: peripheral IV starts, PICC/CVC dressing change, port-a-cath access/deaccess, SQ infusion, pump programming/reprogramming, line maintenance, declotting of central line, lab draws from peripheral and central lines and suture removalTimely documents observations and information according to ministry/HCC policies and proceduresPrepare clinical and progress notes within the specified time frameKeep RN Supervisor/Manager and other stakeholder colleagues within Providence Infusion and Pharmacy Services informed regarding changing status/condition of patients and/or other pertinent observations or learned information (e.g.

demographic, insurance changes, etc.)Promote and practices patient/family, personal and environmental safety while performing job duties according to ministry/HCC policies and proceduresAssist in the assessment, planning, implementation and evaluation of patients and family educational needsProvide instructions/assistance in learning appropriate self care techniques and advocates use of appropriate community resourcesParticipate in Discharge PlanningParticipate in educational in-services for self-enrichment to maintain a high quality of nursing practice and seeks further growth through other/outside educational opportunitiesAppropriately cleans and maintains the cleanliness of the care environment and fixtures, furniture and equipment within the ambulatory infusion suite and related/coordinating areasQualifications:Required qualifications for this position includes:Current licensure as a Practical/Vocational Nurse in the state(s) in which they practiceCurrent State Driver’s license in state in which they reside and have access to daily use of an insured, reliable vehicleCurrent Healthcare Provider CPR certificationPreferred qualifications for this position include:One (1) year of Infusion, hospital, nursing home, or home health experience.About Providence in OregonAs the largest healthcare system and largest private employer in Oregon, Providence offers exceptional work environments and unparalleled career opportunities.The Providence Experience begins each time our patients or their families have an encounter with a Providence team member and continues throughout their visit or stay.
