Vibra Specialty Hospital of Portland is seeking an experienced Case Management Extender – LPN/LVN – to join our team!
The Case Management Extender is responsible for participating in a collaborative process to monitor and evaluate options and services to meet an individual’s health needs. Through communication and available resources, provides quality, cost-effective care utilizing selective benefits and alternative services best suited for the patient, while ensuring optimal outcomes. Key in ensuring appropriate sequencing of treatment goals and implementation of a comprehensive discharge plan. Demonstrates knowledge and skills necessary to provide case management services.
Job Requirements:
- Current, valid, and active license to practice as a Licensed Practical/Vocational Nurse a Bechelor’s or Master’s degree in Social Work (BSW or MSW) in the state of employment required.
- Current BLS certification from a Vibra-approved vendor required.
Additional Qualifications/Skills:
- Experience in a rehabilitation or acute care setting preferred.
- Proficient in collecting and retrieving material from medical records.
- Ability to project a professional image.
- Knowledge of regulatory standards and compliance requirements.
- Strong organizational, prioritizing and analytical skills.
- Ability to make independent decisions when circumstances warrant.
- Working knowledge of computer and software applications used in job functions.
- Freedom from illegal use of and effects of use of drugs and alcohol in the workplace.
