Sapphire Health Services is a local senior care provider since 2011 that is dedicated to providing the best quality of life to each person we serve.
Sapphire is seeking a self-driven Licensed nurse to be responsible for the daily care needs of each resident in their designated section of the facility.
The nurse will assist with providing primary care as well as provide more complex medical care, while support resident in their home environment.

PRIMARY RESPONSIBILITIES:
General Management DAILY
Provide primary care to residents with varying backgrounds and medical histories.
Assess residents for change of condition.
Provide medical intervention as needed for emergent needs.
Assist with the new resident admission process.
Communicate needs of the resident to outside providers including but not limited to: Primary Care Provider, Home Health Agency, Pharmacy, etc.
Communicate resident condition to family, and or responsible party members.
Implement and follow Primary Care Provider’s orders.
Provide medications and treatments per PCP orders.
Complete incident and accident investigation in collaboration with other clinical personnel.
Assure care planning updates, temporary or long term as needed.
Assist other clinical personnel to identify condition changes and complex medical add on residents.
Maintains a professional demeanor in the midst of verbal or physical aggression.

Requirements:
QUALIFICATIONS/SPECIFICATIONS:
1.
Genuine concern for and ability to work with the elderly.
2.
Knowledge of the requirements for providing care and supervision to the elderly.
3.
Ability to direct care.
4.
Ability to manage and prioritize a large, complex workload.
5.
Possess sufficient communication and language (orally and written) skills to perform job duties and communicate with residents, other staff, family members, etc.
as needed.
6.
Ability to make sound decisions when faced with ambiguous situations and to function effectively despite conflicting information.
7.
Must be able to utilize standard precaution knowledge and infection control measures where required.
8.
Knowledge of and ability to conform to the applicable law, rules, and regulations.
EDUCATION:
Must maintain an unencumbered Nursing license from the state in which the facility is located.

PHYSICAL JOB REQUIREMENTS:
In a typical eight hour day, the employee will:
Stand/walk frequently, punctuated by opportunities to sit.
Sit frequently, up to 1-2 hours at a time, punctuated by varying intervals to stand.

Occasionally kneel, bend, and reach while filing paperwork for items = 10#.

Secure proper assistance for transferring and lifting of residents as needed (training required for this task).
Vision
– must be able to read clearly with or without corrective lenses.
Hearing – must be able to hear the telephone, audible alarms, bells, and signals related to resident safety, with or without hearing devices.
Speech/language – must have a strong command of the English language sufficient to read and write and interpret medical information, contract, and administrative information.
