Social Worker (caregiver Support Program)

SummaryThis position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program.

Learn more Learn more about this agency ResponsibilitiesThe Social Worker Caregiver Support Program Coordinator for the Program of Comprehensive Assistance to Family Caregivers (PCAFC) works in the Social Work Service and will be located at either the Roseburg VA or at the Eugene Community Based Outpatient Clinic.

The duties of this position include but are not limited to the following:Provide clinical evidence-based services and interventions; program development; caregiver, Veteran and staff education on caregiver issues; community outreach; resource development; continuous quality improvement activities and evaluation/consultation related to the PCAFC.Function in support of caregivers, Veterans and family members throughout the Medical Center and in the community and will support the development and implementation of General Caregiver services.Working with the facility CSP Program Manager to provide program support and oversight of the PCAFC under the umbrella of Caregiver Support Program services.Leading CSP PCAFC staff, tracking consults and workload, assisting, covering, conducting frequent needs assessments to implement education, wellness, and support groups.Must be a skilled communicator capable of developing effective partnerships with local, state and federal agencies as well as non-governmental organizations.Administratively responsible for the clinical programming and daily operations of the Program of Comprehensive Assistance to Family Caregivers.May serve as back-up to the facility CSP Program Manager.Assists with the development of policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use.May be responsible for the program’s budget and various control points assigned to the program within the assigned areas.Manages and controls the use of data related to these programs regarding day to day functioning and program evaluation.Plans, organizes and directs administrative, operational, caregiver, Veteran and personnel activities for the program.Demonstrates the advanced practice skill level in social work.Will function in support of caregivers, Veterans and family members throughout the medical center and in community settings.Administer a facility PCAFC.Provide consultation and guidance to colleagues., role model effective social work practice skills, teach or provide orientation to less experienced social workers.Develop innovations in practice interventions.Provide clinical supervision for social work licensure or certification.Requires advanced knowledge and understanding of the services and benefits available for caregivers from both federal, state and private sector resources.Assigned to work independently with multiple VA healthcare services and with community agencies, leaders and organizations where he/she has no access to direct social work clinical supervision.Responsible for developing and establishing linkages and collaborates on VA caregiver support efforts with community healthcare and social service agencies.Administratively responsible and accountable for the clinical and program development and effectiveness of the facility PCAFC.Adhere to the NASW Code of Ethics, all local social work policies, and maintain an unrestricted, unsuspended license to practice social workWork Schedule: Monday through Friday, 8:00 a.m.

Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency.

Candidate may have certification or other post-master’s degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.(2) Licensure/Certification.

Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.(3) Demonstrated Knowledge, Skills, and Abilities.

In addition to the experience above, the candidate must demonstrate all of the following KSAs:(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.(c) Ability to supervise multidisciplinary staff assigned to the program.(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).(e) Ability to provide training, orientation, and guidance within clinical practice.References: VA HANDBOOK 5005/120 PART II APPENDIX G39The full performance level of this vacancy is GS-12.

Physical Requirements: Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies.

Applicants must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation if necessary, without endangering the health and safety of the applicant or others.Position requires walking throughout the clinic and the Medical Center, standing for extended periods of time, and involves sitting at a computer and in counsel and consultation with Veterans and interdisciplinary teams.Position may require minimal travel into the community to meet with community agencies and partners who assist and support to the Caregiver Support program, its Veterans and caregivers.This position also requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers.
